As a Urology Medical Record Coder, you will perform all medical record coding activities, ensuring the accurate assignment of diagnostic codes to patient charts and reports.
Responsibilities
+ Assign appropriate diagnostic codes to patient charts and reports.
+ Abstract relevant clinical and demographic information from the medical record to assign ICD-10 and CPT-4 codes in accordance with coding and reimbursement guidelines.
+ Identify principal and secondary diagnoses with minimal error based on national standards.
+ Code with an accuracy of 95% based on QA internal reviews.
+ Record all diagnostic procedures and assign appropriate procedure codes.
+ Request diagnosis from physicians when information is not recorded.
+ Determine and record the required medical information.
