The Transition Facilitator at Lakeridge Health plays a crucial role in ensuring excellent patient-centered care during transitions. Reporting to the Manager of Transition Management, you'll collaborate with healthcare professionals, patients, and families, removing barriers and facilitating thorough support throughout the transition process. Your knowledge of community resources and legislation will be instrumental in reducing unnecessary hospital stays.
