Key Responsibilities

  • Evaluate and treat patients in a skilled nursing facility (SNF) setting
  • Develop and implement individualized physical therapy treatment plans
  • Monitor and document patient progress toward functional goals
  • Collaborate with interdisciplinary care team to coordinate patient care
  • Educate patients and caregivers on therapeutic exercises and home programs

    • Required Qualifications

  • Active Physical Therapist (PT) license in the state of Wyoming or ability to obtain prior to start
  • Graduate of an accredited Physical Therapy program
  • Current CPR/BLS certification
  • Experience working in a skilled nursing or long-term care setting
