Job scope:

. Build strong relationships with patients, caregivers, and healthcare professionals
. Coordinate and support patient care activities using healthcare IT systems
. Track, document, and monitor patient care interventions and clinical indicators
. Perform basic health parameter measurements (e.g. blood pressure, weight, glucose monitoring)
. Support national preventive health screening and vaccination initiatives
. Provide basic health education on lifestyle management, physical activity, and healthy living
. Promote social prescribing and increase awareness of community resources available to patients
. Educate patients on the use of health monitoring devices and vaccination requirements
. Support efforts to reduce hospital readmissions and improve patient outcomes through preventive car
