In the Community Complex Care Team we provide patient and family‑centred care in a variety of settings. The role requires working within the Stoke‑on‑Trent community, covering a 24‑hour period, and includes days, nights and unsocial working times. You will work with one client throughout your shift, providing high‑quality, complex care.

Full training will be given to meet the individual's needs, but previous care experience is required.

Main duties of the job

• Deliver high‑quality care in patients' homes or designated settings, including management of tracheostomies, ventilators, enteral feeding, complex seizures and medication administration.
• Assess patient care needs daily, implementing and evaluating care plans produced by qualified nurses with patients and their families.
