We’re seeking an experienced Support Coordinator to join to help us build and deliver this important part of our business.

About the Role
In this role, you’ll work directly with NDIS participants, families and service providers to coordinate supports, build capacity and ensure participants get the most from their plans.
You’ll also collaborate closely with our internal nursing and support teams, assisting with service delivery coordination to ensure smooth communication and holistic care.
