Social Worker

Overview

The Social Worker is responsible for providing professional, person-centred psychosocial support and case management services to individuals living with dementia and Alzheimer’s disease, as well as their families and caregivers, across Mpumalanga Province. Reporting to the Regional Manager/Social Worker, the incumbent conducts comprehensive assessments, develops and implements intervention plans, facilitates support groups, delivers community education and dementia awareness programmes, maintains accurate client records, and prepares reports. The role requires extensive travel throughout Mpumalanga to provide services in communities, healthcare facilities, residential care homes, and partner organisations.
