The Social Worker II will work within the multi-disciplinary Home Living team, providing a wide array of social work services to patients and families. Your role includes conducting psychosocial assessments, managing transitions, and offering vital resources and support, ensuring tailored care for each individual.
Key Responsibilities:
• Conduct individual psychosocial assessments for clients
• Implement relevant resource-centric interventions
• Facilitate treatment in individual, group, and family settings
• Rigorously screen for risks and coordinate cases
• Assist with transition and discharge planning needs
Requirements:
• Must possess a Bachelor’s degree in Social Work
• Active or eligible for registration with ACSW
