Duties and Responsibilities

• Provide short to long term on‑site and in‑home intensive case management support to individuals and families who may be vulnerable, at risk or experiencing homelessness.
• Create and update case plans on a regular basis.
• Provide crisis intervention and clinical counselling using various therapeutic modalities.
• Assess bio‑psychosocial needs and make referrals to appropriate community resources.
• Complete suicide, homicide and other risk assessments, and where appropriate co‑create safety plans with clients.
• Complete applications to the Justice of the Peace for Form 2 – Order for Examination under s.16 of the Mental Health Act where applicable.
• Coordinate and facilitate interdisciplinary case conferences for high acute individuals with complex needs with internal and external sector stakeholders in health, justice, housing, homelessness, social services and others as necessary.
