Job Description
- Conduct assessments of needs, strengths and challenges of individuals and families in the community through comprehensive interviews.
- Develop and implement individualized service plans in collaboration with clients.
- Provide counseling, emotional support and crisis intervention to individuals and families.
- Advocate clients within the community, ensuring access to services, resources and opportunities that promote self‑sufficiency and well‑being.
- Collaborate with stakeholders, community organizations and government agencies to address systematic issues, develop partnership and enhance service delivery.
- Conduct outreach activities and educational workshops to raise social awareness of social issues.
- Maintain accurate and up‑to‑date records of client interactions, assessments, service plans, progress notes and referrals in accordance with agency policies and legal requirements.
