• Services Coordinator Department:  Developmental Disabilities Services Salary: $26.72 - $28.92   General Position Summary The position provides targeted case management services to individuals experiencing Intellectual/Developmental Disabilities (I/DD) by identifying individual needs and developing plans to access available services and resources.


• Essential Functions The following may not be all inclusive and other duties may be assigned.  Assess and periodically reassess an eligible individual to determine service needs.
• Develop and periodically revise an ISP or Annual Plan based on information collected through an assessment or reassessment that specifies the desired outcomes, goals, and actions to address the medical, employment, social, educational, and other services needed by the eligible individual.
