Service Coordinator (KRCC Northern Counties - Breathitt, Lee, Owsley, or Wolfe based)

Title:

•  Service Coordinator Company/Location: Kentucky River Community Care/Can be based in either Breathitt, Lee, Owsley or Wolfe Counties Employment type:  Regular Full Time Credentials Required: Bachelor's degree in approved Human Services field (Social Work, Psychology, Sociology, etc.) with one (1) year post graduate work with children.

Special Skills/knowledge:

•  good communication skills, maintain client related goal data, including case notes, outreach, and relationships with community partners.

Primary Functions:

•   Provide case management services including needs assessment, service plan and monthly contacts.
