SCOPE Project Coordinator

OVERVIEW

This position is a contract position until October 31, 2026 with possible extension to oversee and carry out implementation of the East Toronto Seamless Care Optimizing Patient Experience (SCOPE) Program, administered by the East Toronto Family Practice Network (EasT-FPN) and Michael Garron Hospital (MGH).

SCOPE is a virtual, interdisciplinary program connecting primary care providers with hospital services and community health partners to improve patient care and access.

The is the Primary Care Network within the East Toronto Ontario Health Team (East Toronto Health Partners).The mission of the EasT-FPN is to represent the voices of primary care providers (family physicians and nurse practitioners), in East Toronto, through a collaborative OHT decision-making structure.
