The Supervisor, Care Management oversees daily operations and staff within the Care Management department to ensure timely, safe, and cost-effective patient transitions throughout the continuum of care. This role supports care coordination, utilization review, and discharge planning processes in collaboration with physicians, nurses, and other interdisciplinary partners.
