This position assesses, plans, implements, evaluates, and documents nursing care of patients in accordance with organizational policies. The role is accountable for the quality of nursing services delivered, utilizing professional nursing practice standards. The position utilizes specialized knowledge and judgment necessary to assess data and plan care for assigned patients. Essential functions include assessing physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs of patients. Reviews patient history and ensures completion within an appropriate timeframe. Completes assessments and reassessments according to patient needs and policy. Formulates a plan of care, including discharge planning, using assessment data and input from the patient, family, and health team.
