Certifications Required: BLS

NO CLINICAL INVOLVEMENT



Home Health & Hospice experience required.

Must have current Driver's License & Auto Insurance



- Common diagnosis / Types of patients/patient demographics: Home Health and Hospice Care. Adults, Geriatrics, Peds

- EMR HomeCare HomeBase

- Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?): a-p. On call rotation for evenings/weekends

- Weekend Requirements: On call rotation

- Holiday Requirements: Holiday coverage required

- Job Responsibilities:

Initial assessment of client and family to determine home health needs - complete physical assessment & history of current & previous illness.
