Amergis Healthcare is actively looking for a **Case Manager** RN that would be interested in an **13 Week** **,** **full-time contract** opportunity in **Elmhurst, IL** . Additional details of the contract are below:
**Position:** RN - Case Manager
**Location:** Elmhurst, IL
**Pay (local):** $45/hr
+ **$0.725 per mile reimbursed**
**Schedule:**
+ Full Time
+ Monday - Friday (open to Sunday - Thurs)
+ 8:30 AM - 5 PM
**Duration:**
+ 13 Weeks (temp to perm)
**Requirements:**
+ **2+ year of experience as Case Manager RN**
+ **Experience with home visits or community visits**
+ **hospice experience REQUIRED**
+ **assisted living, and independent living experience preferred**
+ **Active Illinois Registered Nurse licensure**
+ **Travel Location could be Northbrook, Park Ridge, Skokie, Oak Brook, La Grange**
