Remote Cardiology Patient Navigator & Care Coordinator

Seamless Assist is looking for a Remote Patient Care Coordinator to support our US Cardiology Team. This position will manage patient communication, scheduling, and documentation for a caseload of 150+ patients. You will ensure patient care continuity while working in remote settings during US business hours (Central Time).

The ideal candidate will have a clinical healthcare background (CNA, MA, RN), 3+ years of experience in healthcare, and strong communication skills. This role is pivotal in providing high-quality patient care in a remote environment.
