This full-time, temporary position requires you to assess and understand the health concerns of new patients, helping them connect with the right services at NFCHC. The RPN Intake will create a nurturing and culturally safe atmosphere while collaborating with healthcare professionals and community organizations to promote client-centered care.
Key Responsibilities:
• Serve as the first clinical contact for new patients
• Conduct comprehensive assessments using standardized tools
• Determine care suitability for CHC or referrals to external providers
• Coordinate referrals to internal and external services
• Track patient attachment status in EMR documentation
