Unit Summary

The Integrated Stroke Model of care provides assessment and treatment to stroke patients along the continuum. The stroke system begins in acute care on the 24 bed unit on 7South at the Hamilton General Site. Stroke recovery continues on B2North, a 28 bed unit in the Regional Rehabilitation Centre co-located at the HGH site. The team also works closely with community partners to support ongoing integration into the community. Staff will have experience with skills in the treatment of stroke patients in both the acute and rehabilitation phases of the care continuum to support quick and seamless transitions between units. This unique model provides improved access to best practice and rehabilitation care for all individuals who have suffered a stroke.
