JOB SUMMARY

The Extra-Mural Program Hospital Liaison Nurse is an integral member of the interdisciplinary team, responsible for coordinating care between hospital and home at both the local and provincial levels. Guided by the philosophy, objectives, and policies of the EMP, the Liaison Nurse determines eligibility based on the EMP mandate, clinical criteria, and service scope. This includes completing a bedside assessment of each patient’s medical, functional, and social needs to support safe, effective, and timely discharge planning.

Key responsibilities include coordinating EMP clinical services, reducing unnecessary hospital admissions through proactive care planning, and educating hospital personnel about the scope and limitations of the EMP. The Liaison Nurse also engages patients and caregivers in developing care plans, provides information about EMP services, and assists with short-term equipment needs within program guidelines.
