This role leads the team that works with NH leaders, providers and staff to change to more consistent, evidence-based clinical processes, practices, documentation, and patient/client information, enabled by digital health tools and Clinical Information Systems (CIS). The team also works to increase the effective and meaningful use of EMRs through data-informed coaching, human change management, learning strategies and networks of peer mentors.
