Job Description
What you’ll do:
- Build trusting, respectful relationships with people accessing CPS support
- Develop and deliver individualised recovery plans focused on people’s strengths, goals, and needs
- Support individuals through the NDIS access and planning process, including gathering evidence and attending meetings
- Help people build their capacity for daily living, social interaction, education, and employment
- Link individuals with other services, supports, and local opportunities
- Collaborate with families, carers, and service providers to support holistic outcomes
- Work up to a fixed 75-hour fortnight (Monday to Friday, business hours)
- Use your own vehicle to travel between participants (mileage reimbursed). Most travel will be within 30 minutes of each location.
