Practice Pharmacist

Longton Medical Centre is looking for anenthusiastic clinical pharmacist to join our expanding team. We are awell-established training practice with a growing list size ofapproximately 6400 patients.

Our current pharmacy team comprises of 1pharmacist who is supported by pharmacists employed by our localPrimary Care Network (PCN). We also work closely with our Medicines Managementcolleagues.

The post holder will work alongside otherclinicians to provide primary support to practice staff with regards toprescription and medication queries. They will help manage the repeatprescription system, deal with acute prescription requests, and medicinesreconciliation. They will advise on safe prescribing within the practice. Theywill undertake medication reviews to proactively manage patients includingthose with complex polypharmacy. Conduct safety audits as per CQC requirements
