Job Description
We are looking for an experienced registered nurse (RN), physiotherapist, occupational therapist, or social worker (RSW) who can provide connected, accessible, patient-centered care as a Placement Coordinator. You will play a lead role in supporting patients to be healthier at home while benefiting from our supports for growth, personal wellness, and work-life balance.
Reporting to a Senior Manager, the Placement Coordinator is responsible for reviewing client assessments and eligibility, authorizing admissions to Long Stay, Short Stay, and Convalescent Care beds in Long Term Care Homes (LTCH), and managing Long Term Care waitlists.
Key accountabilities
- Obtains and reviews applications for long-term care in both in-home and hospital settings.
- Completes assessments using RAI-HC.
