Overview
An exciting opportunity has arisen for an experienced HCSW to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS). The Intermediate Care Service provides rehabilitation to the adult population within the locality, delivered in patients' own homes or as out‑patients.
Responsibilities
- Work with the Physiotherapy Service within Intermediate Care to deliver therapeutic treatment programmes to service users in their own homes, preventing hospital admission and supporting early discharge from hospital.
