Job Description

Duties:

  • Patient Access & Triage: Managesame-day appointments, virtual consultations, and patient queries; assess
    acuity, offer counselling on health promotion, and arrange timely follow-up.
  • Clinical Assessment & Diagnosis:
    Conduct patient histories and physical examinations; develop differential
    diagnoses and document all encounters in the electronic medical record (EMR).
  • Investigations & Treatment: Order
    and interpret lab tests and imaging; initiate, renew, or adjust treatments
    within the PA’s delegated authority; coordinate referrals to external providers
    and community supports.
  • Chronic Disease Management: Lead or
    support clinical programs for diabetes, hypertension, COPD, mental health, and
    other common primary-care conditions; provide lifestyle coaching and medication
    education.

    permitted under medical directives (e.g., immunizations, wound care, Pap tests,...
