**Physical Therapist (Home Health)**

**Job Type:** Full-Time
**Schedule:** Monday - Friday | 8:00 AM - 5:00 PM
**Compensation:** $135,000 - $149,000 annually (Pay Per Point Model)
**Sign-On Bonus:** $20,000
