Job Description
- The Physical Therapist (PT) evaluates, plans, and delivers evidence‑based physical therapy interventions to patients across acute hospital, acute rehabilitation, inpatient, medical‑surgical, and outpatient care settings
- Conducts comprehensive assessments, develops individualized treatment plans, and provides therapeutic interventions aimed at improving mobility, strength, pain management, and overall functional independence
- Collaborates closely with the interdisciplinary care team, participates in discharge planning, and educates patients and caregivers on home exercise programs and safety strategies
- Ensures accurate, timely documentation and maintains compliance with regulatory and facility standards while delivering high‑quality, patient‑centered care
- Additional duties may be assigned as needed
