Job Title: Physical Therapist - Home Health
Location: Epping, NH 03042
Duration: 3+ months (possible contract to hire)
Shift: 8:00 AM – 4:30 PM

Job Responsibilities

  • Provide physical therapy services within the assigned care center territory
  • Meet productivity expectations as defined for the role

    • Required Qualifications

  • Master’s degree
  • Professional certification required

    • Additional Information

  • Weekly hour guarantee: 32–40 hours
  • Productivity expectations: 30 points / 40 hours
  • Coverage area: Epping Care Center territory
  • Mileage reimbursed between visits from first patient to last patient
  • Nice to have: Home Care Experience, HCHB EMR
