Pharmacist

DUTIES AND RESPONSIBILITIES:

The Ontario Renal Network's (ORN) funded Renal Pharmacist role, will play a critical role in ensuring medication safety in the treatment of patients with chronic and acute kidney disease. The Renal Pharmacist is required to support renal patients that are admitted into hospital, support our pre-dialysis patients in clinic, provide expertise for our home hemodialysis and peritoneal dialysis programs and provide on-going care to patients in our in-centre hemodialysis units.

In order to support our Regional Nephrology Program, the Renal Pharmacist will:

* Work within a multidisciplinary team to meet the medication related needs of patients which will include distributing medications, maintaining records and medication profiles
* Help obtain medication coverage when needed;
