Summary In this position you will serve as the program manager for clinical pharmacy services that specialize in drug monitoring and management of therapy, with emphasis on long term care. The Clinical Pharmacist Practitioner (CPP) in Long Term Care provides specialized clinical pharmacy services to Extended Care and other medical center long term care obligations.
