Patient Focused Care Responsibilities

• Deliver clinical services such as medication reviews, chronic disease management and care plan development.
• Administer vaccines and immunizations.
• Determine appropriateness of drug therapy and monitor effectiveness, side effects, and interactions.
• Counsel patients on prescriptions, over-the-counter products, and health-related topics, including indications, contraindication, side effects, and proper usage and storage.
• Supervise pharmacy students or provisional pharmacists in performing restricted activities.
• Ensure compliance with all federal, provincial and regulatory standards governing pharmacy practice.
• Collaborate with other healthcare professionals to optimize patient outcomes.
