This is an exciting opportunity to work with adults of all ages to improve occupational outcomes within the community setting. You will join a supportive community occupational therapy team with line management and supervision from a Community Occupational Therapy Team Manager, and mentorship and support from Senior Practitioners. 

The focus of the role is working with people to increase independence and/or enable safe care within the home environment. You will carry out outcome focused assessments leading to plans for effective intervention. This may involve complex equipment and/or adaptations. You will work within the parameters of eligibility criteria. You will be responsible for a caseload with autonomy to prioritise and manage your daily workload.

You will complete accurate case recording, including reports to evidence need where necessary.
