Responsibilities

• Assess and build accurate therapy care plans based on clients’ home environments, including developing health promotion programs, prescribing equipment and supplies, and promoting client safety.
• Use critical thinking and creativity to provide complex preventive or corrective actions for a variety of clients with diverse needs, solving problems that interfere with their ability to do things that are important to them.
• Partner with clients’ family members, caregivers, and other health care professionals regarding treatment goals and service plans.
• Work within a hybrid model of care, including face‑to‑face and virtual sessions, to build a schedule that works for you and your clients.
