Overview

General Visiting Registered Practical Nurse (RPN) role in community nursing. Scheduled visits to care for patients of all ages and varying complexity of care needs. Morning shift 8am to 4pm on weekdays and alternating weekends. Replacement position. Signing bonus offered.

Responsibilities

  • Using appropriate assessment tools/techniques to assess patients’ individualized needs.
  • Executing and continuously monitoring care plans.
  • Engaging patients using a collaborative approach by building rapport and trust.
  • Maintaining and updating professional patient records related to patient encounters and plans.

Qualifications

  • Registration with the CNO as a Registered Practical Nurse in good standing.
  • Valid driver’s license and access to a vehicle.
  • Experience in delivering community care is an asset.
