Location: Chesapeake, VA / Norfolk, VA (Onsite across 3–8 dialysis centers) Schedule: Full-time, Monday–Friday | Daytime hours (8 AM – 5 PM) Work Type: Onsite (no remote option)

As the founding NP in this market, you'll shape a first-of-its-kind initiative that integrates primary care, nephrology, and chronic disease management —bringing care to patients where they already receive dialysis. This is a clinical leadership role with significant autonomy and opportunities for innovation and future advancement.

Key Responsibilities

• Deliver comprehensive primary care to patients within assigned DaVita dialysis centers (10–12 patient visits per day).
• Manage chronic health conditions including diabetes, hypertension, COPD, heart failure, and other ESRD-related complications.
