The Judge Group is Hiring: Per Diem Nurse Practitioner / Physician Assistant
Location: Herkimer County, New York
Update:
Preferred Experience: Primary Care Provider (PCP) or in‑home health experience; ideally both.
Minimum Monthly Requirement: 15 completed visits.
Providers must be credentialed with Excellus, NYS Medicare, and Medicaid to avoid credentialing delays.
Travel Requirement: Up to a 50‑minute radius.
Position Summary
The Provider performs in‑person, no‑cost annual preventive health risk assessments for Medicare, Medicaid, and Affordable Care Act members. Assessments take place within the member’s personal living environment and must meet all health plan expectations and quality standards.
