As a Contractual Mentor, you'll provide individualized support to youth enrolled in the Alternative Placement for Youth Program (APY). Your responsibilities will include working one-on-one with participants to enhance their social competencies and life skills. You'll engage in practical activities while documenting interactions to ensure continuity and effectiveness of support.
Key Responsibilities:
• Develop personalized plans with participants
• Offer transportation as required
• Conduct structured activities promoting life skills
• Record interactions in an Electronic Medical Records system
