MDS Coordinator (RN)

RN MDS Nurse Creekside Post Acute Care Center in Yucaipa CA, is currently seeking a highly skilled and experienced RN MDS Coordinator to join our team.

As an MDS Nurse, you will be responsible for coordinating the development and completion of resident assessments in accordance with federal and state regulations.

You will also collaborate with other members of the interdisciplinary team to ensure that care plans are accurate, individualized, and reflect the needs and preferences of each resident.

Job Responsibilities:
