Job Description
Summary
The Licensed Social Worker LSW is a member of the Quality Management team and collaborates with members of the interdisciplinary team (patient, family, provider, RN Case Managers, nurses, therapist, etc) in case finding/screening, assessment, planning, implementation, evaluation, monitoring and documentation of the patient’s care, psychosocial, and discharge needs.

Required Minimum Knowledge, Skills, And Abilities (KSAs)
- Education: LSW Social Worker and/or LSW Social Worker with MSW supervision if indicated by regulations with at least two years of experience, preferably inclusive of experience in an acute care or Clinical Social Worker required.
- License/Certifications: Current license with the Maine State Board of Social Work required.
