Summary The Patient Aligned Care Team (PACT) LPN is responsible for delivering clinical and clerical support to the PACT healthcare team in accordance with established policies, procedures, and protocols. The PACT LPN works collaboratively with the healthcare team to provide patient-centered care to veterans, ensuring efficient and effective clinical operations. Position is at the Monongalia County VA Clinic in Westover, WV Responsibilities Duties include but are not limited to: Held accountable for delivering direct patient care from adults through geriatric Veterans for both acute and chronic diseases in an outpatient setting. Implement basic and advanced LPN procedures as defined by policies and procedures. Completion of pertinent clinical reminders and preventative health screenings.
