REPORTS TO: Director of Nursing Services (DON)
JOB SUMMARY: The licensed practical nurse is responsible for making decisions that are based on the individual's educational preparation and nursing experience, and for providing nursing services as ordered on patient plan of care. Reports problems/potential problems to RN/Case Manager.
DUTIES AND RESPONSIBILITIES:

1. Participation in the nursing process by providing input to the clients plan of care
2. Implementing the care plan as written
3. Evaluating effectiveness of care plan, make recommendations for revisions as appropriate.
4. Records all pertinent observations and treatments as per agency policies. Prepares clinical progress notes and provide appropriate documentation of service in keeping with agency policies and procedures.
