We are looking for a compassionate LPN with the Hospice Heart to join our care team who are committed to providing both medical and emotional support to patients and families.

  • This is an evening position 5x 8 hour shifts (4:30pm-12:30am) - Rotating Weekends & Holiday 
  • Position covers Suffolk County
