Licensed Practical Nurse

We are seeking a Licensed Practical Nurse.  Memory Care experience Is strongly preferred.   The LPN is responsible for providing medical treatment and personal care to residents in accordance with state regulations, company policies, and the Nurse Practice Act.
Work under the direction of the Health Care Coordinator (HCC) to ensure resident safety, comfort, and well-being.

Key Responsibilities:

• Administer medications and treatments; document accurately in EHR.


• Take vital signs and monitor resident condition.


• Assist with resident assessments, nursing calls, and incident reporting.
