Job overview

The post holder will work closely with the community matron to plan, allocate and review patient care. Caseload management is an integral part of this role to ensure that patients are receiving care from the most appropriate service to meet their needs. The post holder will lead on and participate in the assessment, implementation and evaluation of care and be committed to the development of the Primary Health Care Team. He/she may be designated to take charge of the team in the absence of the community matron. He/she will be expected to work across Braintree ICT and, if required, to support other bases within the team.
