Position Summary
Reporting to the Manager, Comprehensive Intake and Care, the Intake Coordinator is responsible for the overall tasks involved with client intake, conducting interRAI CHA pre-screener assessments, communicating effectively with internal and external resources, collecting statistical information, maintaining an accurate, relevant database. The position also provides backup support in the transportation program.
