Local Authority is seeking a Short Breaks Assessment & Support Officer on a 3‑month contract. The role involves assessing needs of disabled children and facilitating access to services and personal budgets in a person‑centred way.

You will work with families, schools and health professionals to tailor support packages and ensure safeguarding and data protection compliance. Immediate start preferred.
