HPB & Transplant Pathway Tracking Coordinator

An exciting opportunity has arisen within the Transplant directorate for a pathway tracking coordinator to join the team. You will navigate patients through their care pathway, from point of referral to treatment in accordance with the national targets. Close attention to detail in updating patients, notes, records and tracking RTT pathways.

Main duties of the job

Responsible for validating, amending and improving data quality in relation to medicine pathways from initial referrals, through diagnostics to completion of patients’ episode. Liaising and co‑coordinating with all involved in the patient’s management.

Accessing and updating EPIC, ensuring that national and local targets are met and reported on at the divisional level and relevant PTLs.
