Responsibilities

To be successful in this role, you will support residents admitted to hospitals within and outside the borough. Your main duties include delivering strength-based preventative support, assessing needs, establishing support systems, and safeguarding patients. You will review care packages and consider placements. As a hospital social worker, you will refer individuals to reablement services to enhance their living skills and ensure timely discharge decisions.
